A little slip of paper that could save your life

Often times when we respond to a medical emergency at a person's home, a senior citizen's center or nursing home there is nobody there, not even the patient, that can provide the paramedics with important medical information.  Information such as current medical conditions, medical history, currently prescribed medications, allergies or even the patient's name or age.

This little slip of paper (see below) filled out and stuck to the refrigerator, the back of the front door, by the phone, or any other prominent location, can provide valuable information that can greatly assist the paramedics in treating the patient, especially if time is critical.
