Assessing

Principal Residence Exemption (PRE) Affidavit	The Principal Residence Exemption Affidavit form is completed when a person purchases a home and intends to use it as their main homestead. The home must be purchased and occupied by May 1 in order for the homestead to be in effect for the current year. Please return this form to the Township Assessing Department.
Request to Rescind Homeowner's Principal Residence Exemption	The Request to Rescind Homeowners Principal Residence Exemption form must be completed when you have sold the property that you have claimed using the Principal Residence Exemption Affidavit. Please return this form to the Township Assessing Department.
Property Transfer Affidavit	The Property Transfer Affidavit must be completed whenever there is a change of ownership on a property. It is completed and signed by the new owner and must be filed within 45 days of the transfer. Please return this form to the Township Assessing Department.
Conditional Rescission of Principal Residence Exemption	This form enables a person who has established a new principal residence to retain a Principal Residence Exemption (PRE) on property previously exempt as the owner’s principal residence, and must be filed with the assessor on or before May 1 of the year of the claim.

Development Services

Building/Engineering

Building Permit Application	Building Permit applications are to be used when structural changes or additions are made to an existing building; for all new construction; permanent and temporary signs; pools; decks; and any sheds over 200 square feet.
Electrical Permit Application	Electrical Permit applications are to be used when electrical work is planned for either existing structures or new construction.
Mechanical Permit Application	Mechanical Permit applications are to be used for all new construction; furnace or air conditioning replacement; installing or replacing humidifiers, de-humidifiers, air cleaners or any other modification of an existing heating.
Plumbing Permit Application	Plumbing Permit applications are to be used for all new construction; additions to an existing plumbing system; installation of an irrigation system; installation or replacement of water heaters or water softeners; or any other modifications of an existing plumbing system.
Residential Rental Registration Form	The Rental Certification Ordinance requires the owners of all residential rental buildings to register those buildings with the Township. Any buildings or apartment complexes that are currently available for or being used as residential rental units are required to file with the Township a Rental Registration Form during the period between August 1, 2010 and October 1, 2010.
Site, Woodland Review and Lot Evaluation Application	Site and Woodland Review is required for new residential construction to confirm the compliance to setback requirements and to ensure that trees are not needlessly removed. Site Review is required for all additions to the footprint of existing homes.
Wetland Delineation Application	Wetland Delineation applications are to be used when the property owner wishes to have the boundaries of a wetland flagged. The presence of wetlands on a property limits the buildable area of the parcel and this process enables the property owner to determine the allowable location of new construction or additions to existing structures.
Fill Permit Application	Fill Permits applications are used when the property owner wishes to raise the grade of a property more than one foot. This review will ensure that adjacent properties are not adversely affected by the change in grade.

ZBA Application Packet (includes instructions)	The Zoning Board of Appeals application and fees are required to request a variance from the requirements of the Township Zoning Ordinance. The instructions provide guidance in following the process. The Zoning Board of Appeals application must be submitted to the Community Planning and Development department. Community Development
Community Development Block Grant (CDBG) Application	Federally funded no-interest loan application for emergency and minor home repairs need by low-income single-family residential homeowners. When completed, this application needs to be returned to the Community Planning and Development department. Click here for more information on the Community Development Block Grant Program.
Urgent Needs Home Repair Program - Application Supplemental	The Supplemental Information Form is required to provide basic statistical information as part of the federally funded Limited Repair Program.
Notification of Lead Based Paint Hazards	The Lead Based Paint Acknowledgement form provides Limited Repair Program applicants with information on the hazards of lead-based paint. A copy of this form must be signed and returned as part of the Limited Repair application process.

Bi Weekly Direct Deposit	Use this form to set up or change an existing direct deposit bank account for your bi weekly check, this is limited to full time employees.

Retiree Direct Deposit	Retirees use this form to set up or change an existing direct deposit bank account for your monthly pension check, this is limited to one bank account only and takes two months to go into effect.

BCBS Master Medical Form	Use this form if you have BCBS traditional or Preferred Provider (PPO) coverage which includes master medical reimbursement.
Flexible Spending Health Care Claim form (FSA)	Use this form to send for your FSA, HRA or Dependent Care claim. You need to complete this form and send it with a copy of your paid receipt directly to Flex Administrators. Address and fax number for submitting the claim is on the form!
Flexible Spending (FSA) Health Care Eligible or not expenses list-Revised August 24, 2011	Check this list to see if your expense is something you can turn into your Flexible Spending (FSA) account for reimbursement. The list has one page of Eligible Expenses and one page of Non Eligible Expenses.

Business Alarm Notice Letter and Contact Update Form	Use this form to update your business or residence alarm contact information with the police department. The Waterford Police Department requires that each household and business within the Township register their alarm system. In the case of an emergency at your residence and/or business, pertinent contact information will be very helpful to Dispatch and the responding officers.
Police FOIA Request Citizen Property Form	Use this form to submit a Police FOIA request Use this form to add additional stolen or damaged property to your existing police report. You must make an original report before using this form.
Online Citizens Police Reporting System	An online service to report any of the following incidents when there is no suspect(s): Harassing Phone Calls Theft Damaged Property Vandalism Identity Theft Lost Property Private Property Accident
